Tuition Remission Registration

This form should be completed by the Human Resources Department of a human service provider agency
and mailed with the appriopriate registration fee to :

Checks must be made out to: Massachusetts Council of Human Service Providers, Inc.
You can submit this form anytime during the academic year, but it only provides access to the program for one academic year that start in the fall and ends with Summer session one (June/July classes). For example, if you register in January 2007, employees will be able to apply for Tuition Remission for Winter/Spring 2007 classes and Summer Session I classes. Tuition Remission cannot be applied to classes that have already been completed, even if it is in the same academic year for you have registered for.
We will send out renewal forms in May for the following academic year.
