This form should be completed by the Human Resources Department of a human service provider agency
and mailed with the appriopriate registration fee to :

Checks should be made out to: Massachusetts Council of Human Service Providers, Inc.
You can submit this form anytime during the academic year of 2007-2008,
but it only provides access
to the program for the Summer Session II 2007, Fall 2007, Winter/Spring 2008 and Summer Session I 2008 .
We will send out renewal forms in May for the following academic year.
